The Basics

Option #3

I wish to SEND human blood, human tissue, and/or human plasma:

  1. Complete the "I wish to SEND human blood, human tissue, and/or human plasma - Option #3" form:

Please Note: Only the academic in charge of the laboratory is authorized to, and may request transfers of UCSD research materials. If this is not the same individual as the Principal Investigator, who will be listed on the form, the academic who is in charge of the laboratory must sign the form as well.

  1. Human Subjects (IRB): If a drug or human blood, tissue, and/or plasma is to be sent by UCSD, make sure that a Human Subjects Protocol has been, or is in the process of being, approved by the UCSD Human Subjects Institutional Review Board (IRB). Make sure that the receiving organization identified on the Material Transfer Process form is named in the Human Subjects Protocol. If not, send a request to the IRB to amend the protocol. Additional information regarding the UCSD Human Research Protections Program/Institutional Review Board (IRB) can be found by calling 858-455-5050.

What happens next? Once the completed form is received, a Material Transfer Agreement (MTA) will be drafted and forwarded to the recipient organization for review and signature. A fully-executed copy of the MTA will then be sent to the UCSD Principal Investigator. The material should not be sent prior to receiving a fully-executed copy of the MTA.

The review and negotiation of the Material Transfer Agreement is conducted concurrently with the approval process for Human Subjects.
